Sarah Johnson Conway is an assistant professor of medicine at the Johns Hopkins University School of Medicine, a practicing hospitalist at Johns Hopkins Hospital, and an administrator holding a number of positions within Johns Hopkins Medicine. She is the first Medical Director of a new Johns Hopkins Hospital Infusion Center, which brings together adult infusion services and a specialized program providing urgent and continuity care for the sickle cell population. She is Associate Medical Director of the Johns Hopkins Medicine Alliance for Patients (JMAP), a Medicare Shared Savings Program Accountable Care Organization serving approximately 39,000 Medicare patients. Finally, she is a Medical Director of Health Services at Johns Hopkins HealthCare. Dr. Conway’s work and publications focus on care delivery models for the highest-need patient populations, transitions of care, accountable care, and high-value care. She collaborates with health system leaders to expand access to urgent infusion-based services minimizing unnecessary ED visits and hospitalizations. She works to improve care coordination across the care continuum, expand palliative care and advanced care planning resources, and streamline and improve transitions of patients from the hospital to post-acute care venues. She also supports efforts to increase high-value prescribing and ordering practices.
Dr. Conway graduated cum laude from Harvard University with a degree in Economics. Upon graduating she worked on Wall Street as an investment banking analyst at Goldman Sachs before finding her passion in medicine and completing a post-baccalaureate program at the University of Pennsylvania.
She earned her M.D. from the Perelman School of Medicine at the University of Pennsylvania and was named a 21st Century Scholar receiving a 4-year full tuition merit scholarship. She completed her residency in Internal Medicine at Johns Hopkins University School of Medicine, completing a Patient Safety and Quality Improvement Pathway. Following residency, she served as the inaugural Johns Hopkins Medicine Health Systems Management Fellow. She is board certified in Internal Medicine. Dr. Conway was raised in Chevy Chase, Maryland and currently resides in Baltimore, Maryland with her newly expanded family.
